In this case, only 2 years had passed since the first facelift, yet there was a considerable amount of excess skin that could be removed. Given that the patient's satisfaction had already declined immediately after the first surgery, we anticipated that redoing the facelift even after such a short interval would still produce a significant improvement.

Despite this being a revision surgery, the tissue elasticity and thickness were relatively well preserved, so the threads inserted during the touch-up procedures were removed. Since the amount removed was not substantial enough to be classified as a foreign-body removal procedure, there were no particular difficulties involved.

Because the effects of the first surgery and the two thread-lifting procedures had minimal impact on this revision, the process was effectively carried out as if it were the patient's first facelift. As anticipated before surgery, there was significant mobility of the fascia and skin, and a substantial amount of excess tissue was able to be removed.

As is occasionally mentioned even in textbooks, it cannot be said that facelift surgery is always performed properly in every case.

This case was one in which we recommended revision surgery because the first surgery appeared to have been little more than a token gesture. Unlike eye or nose surgery, with facelifts it is difficult to immediately perceive a noticeable difference between a properly performed surgery and one that was merely superficial. This is because, due to swelling and the tension from the tightening, everything tends to look fine right after surgery. However, we live under the constant influence of gravity, and due to various factors such as muscle movement and the tissue's own elasticity, the duration for which the results of the surgery last differs dramatically depending on how properly the surgery was actually performed.

This was a case that served as a reminder of the importance of performing the surgery correctly and properly from the very start.
