Why Liposuction Is Not Enough For Double Chin l Fat-Dominant Double Chin Cases

Today, let's take a look at the characteristics of the most commonly seen "fat-dominant double chin"
and how it can be improved, along with actual case examples.

 

A double chin refers to a condition in which, when viewed from the front,
fatty tissue bulges out beneath the chin, making it appear as though there are two chins.
 

With a fatty double chin, the area under the chin
is filled with fat and sags downward, so the contour beneath the chin often appears
prominent not only from the front but also from the side.


 

On ultrasound examination, two thick layers of fat are frequently observed 
above and below the platysma muscle.

Among patients with a fatty double chin, it is common for the outer fat and inner fat layers to be similarly thick.
 

In such cases, liposuction alone - removing only the outer fat
may not bring out as significant an effect as expected.
 

Therefore, by sufficiently removing both the outer and inner fat
and suturing the loosened muscle to bring it into close contact with the area
beneath the chin (preventing skin sagging as the fat is removed), the previously obscured jawline can be restored.





 

In addition, fat-dominant type double chins often involve excess fat in the lateral jaw area
as well, so addressing this region together can reveal the hidden jawline,
resulting in a much clearer facial contour overall.
 

Once the jawline is restored, the face can appear smaller
even from the front, giving an overall more refined impression.



 

This patient has a relatively fuller face overall,
but the double chin stands out even more prominently.

As anyone who has tried dieting knows, it is not easy
to lose weight to the point where facial fat visibly decreases.

Rather than attempting to aggressively reduce overall facial fat,
simply refining the jawline can create a noticeably slimmer appearance overall.


 

From the side profile as well, a clear boundary forms
 between the chin and neck, creating a much cleaner contour.

This is not simply the result of fat removal alone, but rather because
the muscle was also sutured together to lift the area beneath the chin.


 

The next patient does not have an especially severe double chin,
but the area under the chin appears uneven and bumpy.

In such cases, the condition may actually be a muscular-dominant type double chin
rather than a fatty type, which is why ultrasound examination is important.

Ultrasound confirmed that this was a fatty type, so muscle suturing
and liposuction were performed together.
After surgery, the area beneath the chin became naturally flat and tight,
resulting in a neat, well-defined appearance

In some cases, a slight boundary forms under the chin,
but the chin fat protrudes immediately beyond it.
Such patients often have a greater proportion of submuscular fat compared to sucutaneous fat.

Because submuscular fat is relatively less affected by changes in body weight,
many patients feel that the area under the chin does not improve much even after dieting.
 

In these cases, fat removal alone may not be sufficient,
and the loosened muscle must also be addressed together
to genuinely improve the contour beneath the chin.



In the next case, the patient's natural bone structure
should produce a smooth S-shaped jawline contour but due to excess fat and
loosened muscle, the entire area under the chin appears sagging.



In such cases, removing fat alone may not result in a significant change
in the jawline because the loosened muscle cannot properly adhere to the area beneath the chin.

Therefore, combining liposuction with muscle binding
allows a more natural correction of the contour beneath the chin.


 

This is a case of a patient in her 50s.

Normally, as people age, the skin beneath the chin tends to loosen, leading to neck wrinkles;
however, in this patient's case, the excess fat had actually kept the skin taut.


 

However, when she lowered her head down, a loss of skin and muscle elasticity became apparent.

In such cases, removing fat alone can make wrinkles or sagging more noticeable,
which is why properly securing the muscle beneath the chin is an important part of the process.

Doing so allows the double chin to be improved while also minimizing the worsening of neck wrinkles.


 

The final case is a typical fat-dominant type double chin.

Due to the large amount of fat, the skin appears taut and swollen, 

and when viewed from the side, the area beneath the chin protrudes noticeably.

By addressing the fat in the lateral jaw area as well,
the previously hidden jawline can be revealed much more clearly.

If there is no significant change in body weight,the jawline
tends to remain relatively stable over time without major regression.

Fat-dominant type Double Chins are often caused primarily by excess fat and
loosened tissue rather than skeletal or structural issues, making this a type that generally responds well to surgery.

That said, even cases that appear to be fat-dominant type on the surface
may actually be caused by underlying skeletal or muscular issues.
Therefore, it is important to accurately assess the current structure beneath by
}using diagnostic tools such as ultrasound, and to approach treatment according to the underlying cause.
